The District 44 Health Office Staff is committed to taking the best care possible of your child. Please be sure that we have a phone number to reach someone that is available to pick up your child in case of illness or injury. Please keep us aware of any health concerns that your child has.
The following information is an important part of the way we work to care for your child. If your child has any health or medical issues, please share this information so that we can safely care for them at school. If your child becomes ill or is seriously injured during school, every effort will be made to contact you or your emergency contact that you designated on your child's emergency form. If we are unable to reach you in a medical emergency, 911 will be called.

HEALTH EXAMINATIONS AND IMMUNIZATIONS
Preschool, kindergarten, second- and sixth-grade students, as well as all new transfer students, are required to have a health examination and immunizations. Click the drop-downs below to see specific grade-level requirements.
Early Childhood
Proof of a physical examination, including health history completed by the parent, within 12 months of the 1st day of school

Kindergarten

Transfer Students
All new students require proof of health examination by the first day of school and proof of required immunizations per Illinois School code within 30 days of the first day of school.
Any student entering an Illinois school for the first time shall present proof of an eye examination.
Immunizations:
All immunizations must be current according to the requirement of the Illinois School Code. The immunization record is part of the physical but if your child's is separate, you can email or fax it to the school nurse.
For a medical exemption, the student's physician provides written verification of the medical issue requiring the exemption.
For a religious exemption, the student's parents/guardians submit a certificate of religious exemption signed by the student's health care provider.
Health Exam Forms:
PHYSICAL EXAM
Preschool, kindergarten, and sixth grade students, as well as all new enrollees from out of state, are required to have a health examination and immunizations.
EYE EXAM
All children enrolling in kindergarten or in school for the first time are required to have an eye exam.
DENTAL EXAM
Students in kindergarten, second, and sixth grade must present proof of having been examined by a licensed dentist or a waiver within 18 months of May 15th of that school year.
SPORTS PHYSICAL
Students must have a current sports physical to participate in interscholastic sports. The physical must have been performed within one year of the start of the specific sport and must be turned in before tryouts.
MEDICATION PERMISSION
A school medication permission form must be completed by both physician and parent/guardian, and is necessary for prescribed and over the counter medication, including cough drops, that is to be given to a child during school hours. Some medications may also require a care plan. Care plan forms are available below.
Please note:
Medication must be brought to school in the original pharmaceutical container.
Over the counter medication must be brought in its original unopened container.
We will not administer any medication sent to school in baggies, envelopes, etc.
The parent MUST report immediately any changes in prescription or dosage.
New doctor's orders must be obtained for each change.
Medication permission must be renewed at the beginning of each school year.
Cough drops are permissible if they do not contain any ACTIVE ingredients. A parent or guardian must send in the cough drops in the original packaging with written permission for their student to have them. Cough drops that contain ACTIVE ingredients are treated like any other medication. They require parent and doctor written permission. This is an Illinois State Law.
SELF-CARRYING MEDICATION
All medication is stored in the Health Office. If your child will carry and self administer his/her inhaler, their physician must specify this on the medication permission form and as parent or guardian you initial the line giving permission for your child to self carry.
MEDICATION AT THE END OF THE YEAR
All medication must be picked up by a parent or guardian including inhalers unless the doctor and parent has specified that the student may self carry the medication.
Any medication that is not picked up by the last day of school will be disposed of.

Approved foods that may be brought to school for a snack in the classroom:
Fruit (fresh or dried including Raisins & Applesauce)
Vegetables
Pretzels/Plain Popcorn
Goldfish/Graham Crackers/Saltine Crackers/Animal Crackers
Plain Cereal
In accordance with the D44 Wellness and Food Allergy policy, only the foods shown above may be brought to school for a snack in the classroom. There are many students in the building with severe food allergies and because of this, snacks cannot be shared with other students. Snacks that are made in a plant where peanuts and or nuts are processed are not allowed in the classroom.
PHYSICAL EDUCATION EXCUSE
Your child may be excused from PE class for 2 days with a parent note. To be excused any longer requires a note from the doctor.
Please note: NO PE = NO RECESS.
DEALING WITH INJURIES AT SCHOOL
If your child's injury requires the use of crutches or any special instructions, such as extra time in between classes, please have your doctor specify this on his/her care instructions.

Concussions
A concussion is a type of brain injury that changes the way the brain normally works. A concussion is caused by a bump, blow, or jolt to the head. Concussions can also occur from a blow to the body that causes the head and brain to move rapidly back and forth. Even what seems to be a mild bump to the head can be serious. Concussions can have a more serious effect on a young, developing brain and need to be addressed correctly.

Type 1 Diabetes
Type 1 diabetes is a chronic condition in which the body does not produce insulin, a hormone that helps change sugar (glucose) from food into energy. This life-threatening condition causes high levels of sugar in the blood. Without insulin, glucose gets “stuck” in the bloodstream and the body cannot use it for energy. Type 1 diabetes, once known as juvenile diabetes, is the most common type of diabetes diagnosed in children and young adults under 20 years of age.
